  • blue or grey lips or fingertips
  • confused, drowsy or agitated
  • coughing up blood, or have severe chest pain
  • finding your reliever inhaler is not helping at all
  • unable to stay awake, or your family notice you are not yourself

Confusion or drowsiness in someone with COPD can indicate carbon dioxide retention and needs urgent hospital assessment. If you use home oxygen, do not increase the flow rate without advice. Diagnosis

COPD is suspected in anyone over 35 with a smoking history and breathlessness, chronic cough or regular sputum. It is confirmed with spirometry — a breathing test measuring how much and how fast you can blow out. A diagnosis made without spirometry is a guess, and around a third of people labelled with COPD turn out to have something else, most often asthma.

The two things that change the disease

  • Stopping smoking. Nothing else slows the decline in lung function. It works at any age and at any stage, and the benefit begins within weeks. This is the single most valuable thing you will ever do for your lungs.
  • Pulmonary rehabilitation. A structured six to eight week programme of supervised exercise and education. It improves breathlessness, exercise capacity and quality of life more than any inhaler, and reduces admissions. It is consistently under-used and under-referred.

Inhalers

  • Short-acting reliever — for immediate breathlessness.
  • Long-acting bronchodilators — one or two, taken daily, which form the mainstay of treatment.
  • Inhaled steroid — added for people with frequent flare-ups or features of asthma. Not everyone with COPD needs one, and it raises pneumonia risk in some.
  • Technique matters more than the drug. A large proportion of people use their inhaler incorrectly and receive a fraction of the dose. Ask to have it checked at every review — it takes two minutes.
  • Rinse your mouth after a steroid inhaler.

Rescue packs

Many people with frequent flare-ups are given a home supply of antibiotics and steroid tablets. These are for agreed circumstances — usually increased breathlessness with a change in sputum colour or volume — not for any cough. Start them according to your written plan, and tell us that you have, so the pack can be replaced and the flare recorded. Repeated unrecorded courses hide a deteriorating pattern that should be triggering a treatment review.

Recognising a flare-up

  • breathlessness worse than your usual over a day or two
  • more sputum, or a change to green, yellow or thicker
  • increased wheeze, cough or chest tightness
  • reduced exercise tolerance, or needing your reliever much more
  • Act early. Flare-ups treated promptly cause less permanent loss of lung function.

Do

  • Flu vaccine every year; COVID and pneumococcal as advised
  • Keep active every day — breathlessness on exertion is not harmful
  • Learn breathing control and pursed-lip breathing
  • Keep a written self-management plan and use it
  • Ask about a review if you are using your reliever most days

Avoid

  • Smoking and vaping, and smoky or dusty environments
  • Avoiding exercise because it makes you breathless — this deconditions you further
  • Ignoring ankle swelling or worsening morning headaches
  • Running out of inhalers, or relying on a reliever alone
  • Cold air without covering your mouth in winter
