An ingrown toenail happens when the edge of the nail grows into the surrounding skin, causing pain, redness, swelling and sometimes infection or overgrown tissue. It most often affects the big toe. Mild cases can settle with careful nail care; recurrent or infected ingrown toenails usually need a small surgical procedure to fix the underlying problem.
When surgery might be considered
Surgery is worth considering for an ingrown toenail that keeps coming back, is repeatedly infected, or is not settling with conservative care. Importantly, antibiotics alone treat the infection but leave the mechanical problem — the nail edge pressing into the skin — so the cycle tends to restart. Surgery addresses the cause.

A £150 assessment confirms the diagnosis, checks for infection and circulation, and plans the right procedure for you. If you have diabetes or poor circulation, we take particular care and may adjust the plan.
How the procedure works
Under a local anaesthetic toe block, the clinician removes the offending strip of nail — a partial nail avulsion — or, less commonly, the whole nail (total nail avulsion). To reduce the chance of the same nail edge regrowing and causing trouble again, phenol is usually applied to the nail-forming cells (phenolisation). The toe is dressed and you walk out.
Techniques available
Partial nail avulsion with phenolisation — removes the problem nail edge and treats the matrix to reduce regrowth; the usual choice for a recurrent ingrown edge.
Total nail avulsion — removal of the whole nail, for selected cases.
Phenolisation substantially reduces the chance of recurrence compared with simple nail removal, though no procedure can promise it will never come back.

Histology & laboratory testing
Ingrown toenail surgery removes nail rather than a skin lesion, so histology does not usually apply. If overgrown tissue at the nail fold looks atypical, your clinician may send a sample for testing (£120) and will discuss this with you.

Does the procedure hurt?
The toe is fully numbed with local anaesthetic, so the procedure itself is not painful. There may be some soreness afterwards, managed with simple pain relief.
Why won't antibiotics fix it?
Antibiotics treat the infection but not the cause — the nail edge is still pressing into the skin, so the problem tends to return. Surgery corrects the underlying nail problem.
Will it come back?
Partial nail avulsion with phenol greatly reduces the chance of the same edge regrowing. No procedure can guarantee it will never recur, but recurrence is uncommon after phenolisation.
How soon can I walk and return to work?
You walk out the same day. Many people return to desk work within a day or two; keep the dressing dry and wear roomy footwear while it heals over one to two weeks.
Can I have it done the same day?
Sometimes — a straightforward case may be treated at the same visit as assessment when clinically appropriate and the treatment room is available.
